Mysterious death

  Any one knows what happened?? or steps to be taken?
  
  i got killed when i afk. the resulting death is like this.
  
  
  
  very weird cos i still on the platform... i suspect a hack but dunno leh the other guy the kkbird was in the channel b4 i left and he was there when i came back... feels its very weird so came to ask for advice...........
